The field list should also display the field names.

When creating a Notes application using “Create Custom,” you can add fields to forms; however, since only the field labels are displayed in the list, you may accidentally create fields with the same name.
In Notes 14.5 and later, even if there are fields with the same name, no error is generated, and the Notes application is created as is.
This is highly undesirable, so we would like the list to display not only the field labels but also the field names.
This would help reduce the number of instances where fields with the same name are created.
